Development of regulations on investigation of the causes of incidents on hazardous industrial facilities and safety procedures’ approval by related authorities

Accident on a hazardous industrial facility is a demolition of constructions and (or) technical devices and industrial equipment used during operation of a hazardous industrial facility, an uncontrolled explosion and (or) a release of hazardous substances.

Incident on a hazardous industrial facility is a breakdown or damage to technical devices and industrial equipment, deviation from the normal technological process, violation of industrial safety requirements for hazardous industrial facilities.

Subject to compliance with the Art. 9 of the Federal Law No. 116-FZ “On Industrial Safety of Hazardous Industrial Facilities”, an operating organization shall:

• analyze causes of an incident on a hazardous industrial facility, take measures on liquidation of specified causes and prevention of such incidents;
• keep records ofaccidents and incidents on a hazardous industrial facility;
• provide information on the number of accidents and incidents, causes of occurenceand measures taken, to Rostechnadzor.

The procedure of technical investigation of the causes of accidents and incidents is regulated by the Order No. 480 “On Approval of the Procedure of Technical Investigation of the Causes of Accidents, Incidents And Cases of Loss of Industrial Explosives on Facilities Supervised By the Federal Service of Ecological, Technological and Nuclear Supervision” (Rostechnadzor, dd. August 19, 2011).

Subject to compliance with the approved Order (Clause 32), investigation of the causes of incidents on hazardous industrial facilities, as well asrecords and analysis of such incidents and causes, must be conducted in compliance with the document (regulation) approved by the operating organization and coordinated with the regional Rostechnadzor authority..
